About the Project:

Consisting of over 2000 lines of JavaScript, the Lead Sheet digital PDF form was created with the intention of being dynamically filled out based on the end-users/departments requring a file attachment of somekind be added to internal/digital file cabinets for processing.

The form allows for attachments to be made directly to the PDF. Required fields have been setup with notifications if left blank based on selections made by the end-user.Hitting the submit button sets up an email ready to be sent with specific information from the digital form.
